Custom Forms

Collect extra information at checkout and review responses

Custom forms let you ask customers questions during checkout — dietary requirements, t-shirt sizes, workshop choices, and more. Answers appear on the event Responses tab and in order exports.

Configure custom fields

Custom fields are part of the event form:

  1. Edit your event — Go to Events, open the event, and click Edit event.
  2. Open Custom form — In the event form, find the Custom form section.
  3. Add fields — Click Add field for each question. Set label, type, optional description, and whether the field is required. Reorder or delete fields as needed.

For Selection, Choice, and Multiple choice, add options. Each option can have an optional extra price added to the order total.

Delivery settings

In the same section you configure:

  • Delivery method — digital (email PDF) or physical (mailed tickets)
  • Shipping countries — required list when delivery is physical (comma-separated country codes)
  • Accept company orders — show company name field at checkout
  • Disable organizer order notifications — stop email alerts for new orders

Physical delivery requires customers to enter a shipping address at checkout. See Orders.

Customer experience

During checkout, customers see an Additional information section with your custom fields. Required fields must be completed before payment.

Priced options increase the order total automatically.

View responses

  1. Open the Responses tab on the event detail page.
  2. Review answers — each completed order appears as a collapsible entry with customer name, email, date, and answers. Metrics at the top show total responses and field count.

Only completed orders appear here.

Export responses

Form answers are included in order and ticket exports automatically. You can also view responses per order from the Orders page actions menu.
